We have forms available to be filled the day of also. CERF Clinic Presented by Dr Keith Collins of Animal Eye Specialist, Pewaukee, WI If you would like the CERF forms mailed out to you to fill out ahead of time please let me know ASAP.
Deadline for registration is May 7th, 2008 ~All clinics MUST be pre-paid for ~Walk ins welcome if time permits for an additional charge of $5.00 per test. ~Please have your AKC Registration information with you for the paperwork
